الخطوة التالية هي استخراج كرت الرعاية الصحية :

طريقة استخراج كرت SUS Cartao
الاوراق المطلوبه لهدا الكرت بسيطه وسهلة جدا
- جواز السفر الاصلي
- اسم الام والأب ( تكون في صورة من جواز السفر او تقوم بكتابته في ورقة واعطائها للشخص المختص )
- صورة من عقد ايجار البيت حتي يتمكنو من معرفة عنوان السكن
الخطوات :
- البحت علي اقرب مركز صحي ( مستوصف ) ( Posto de Saude ) عن طريق الانترنت ( GOOGLE MAP)
- التوجه الي المركز الصحي في الصباح الباكر 7:30 صباحا هوا موعد فتح ابواب المراكز الصحية
- التوجه الي الاستقبال وطلب استخراج كرت رعاية صحية SUS Cartao يمكنك الطلب بمجرد قولك ل كلمة SUS Cartao او يمكنك كتابة هده الجملة حتي يستطيعو ان يفهمو طلبك Gostaria de extrair um SUS Cartao
- سيتم ارسالك الي مكتب التسجيل في داخل المركز الصحي في حال ان الاستقبال ليس لديهم صلاحية استخراج هدا الكرت.
- بمجرد تسليمك الاوراق المطلوبه سيقوم الموظف المختص بتسليمك الكرت خلال اقل من 5 دقائق
بهده الطريقة تكون قد تحصلت علي بطاقة الرعاية الصحية.
عند استعمالك لهذا الكرت يجب عليك الدهاب في الصباح الباكر 7:30 صباحا حتي يمكنك حجز موعد للكشف .
بمجرد حجزك للموعد يمكنك الحضور قبل الموعد ب ساعه او 30 دقيقه وسيتم النداء عليك باسمك للدخول لغرفة الكشف.
نظام الرعاية الصحية يختلف من ولاية الي اخري ولكن كرت الرعاية الصحية يمكنك استعماله في جميع الولايات في البرازيل دون اي مشاكل .
